District of Columbia Primary Care Association (DCPCA)
DCPCA, serving 200,000 residents through nine FQHCs and community health centers at more than 50 care delivery sites, sought HealthEC’s assistance in 2016 to create an enterprise data warehouse to aggregate data from their EMR, Medicaid claims, and HIE to create a single longitudinal patient record. This would help them view data from all of their care providers, any visit date, any treatments, and all prescription-fulfillment activities.
HealthEC is also working closely with the individual FQHCs to track and improve the outcomes of the patients qualified and attributed to their federally approved Health Home II Project. In addition, we are also helping DCPCA automate their HEDIS reporting to meet the requirements of their FQHC's managed care organizations.
Community Health IPA (CHIPA)
CHIPA, an independent practice association comprised of 23 New York-based FQHCs using six separate EMRs, needed to align its practices and policies in order to succeed with value-based contracts. Rather than rely on anecdotal evidence for care decisions, CHIPA partnered with HealthEC in 2017 to use data and analytics to drive practice transformation.
HealthEC, which has been assisting CHIPA with its first value-based contract with Affinity Health Plan (22,000 adult and pediatric lives), helped negotiate the contract, coordinated the data provided by Affinity, and standardized the reporting requirements to ensure CHIPA and Affinity use identical methodologies to minior progress and performance against the contract.
